Readmission Risk Challenge offers an engaging way to learn about hospital readmission risk factors. With a gaming approach, this app can help sharpen the understanding of what factors increase the probability of a patient needing hospital readmission within 30 days of treatment discharge.

This app uses an approach that categorizes a patient’s risk level of hospital readmission into low, moderate, and high; an approach that some hospital systems use to determine appropriate treatment needs. Users will work against the clock as they aim to correctly identify the risk level and earn the highest possible score. The user scores are presented on the results screen to encourage replay and improvement.
