Description

The sekTOR-HF app is part of the sekTOR-HF innovation fund project of the same name (trans-sectoral needs-based care for patients with heart failure and the development of an alternative remuneration model) and is used, among other things, to record vital data, questionnaires and network patients with medical staff.
The aim of the sekTOR-HF project is to establish a cross-sector care model for heart failure patients (HF) with a telemedical cooperation network. This network is intended to enable centrally located specialists to continuously monitor HF patients so that they can navigate them in good time and allocate resources.
In addition to monitoring vital data and questionnaires in the app, medical assistants will regularly conduct televisits on patients. This post-inpatient care is intended to reduce unnecessary hospital stays and avoid readmissions. Patients with heart failure from the Rhön-Grabfeld and Marburg-Biedenkopf regions can take part.

The care concept of sekTOR-HF and the associated app are based on current scientific knowledge and guidelines for telemedical patient care. We pay attention to the highest possible usability. We regularly test the technical components and continuously optimize our systems and the operating concept. The prerequisites for using the app are a) that an initial adequate diagnosis has been carried out by a specialist for the participating patients and b) that they only use this app with the access data provided to you as part of the sekTOR-HF innovation fund project. The telemedicine platform Curafida, on which sekTOR-HF is based, is certified as a CE-certified medical product of class I according to the Medical Device Directive (MDD).

Please make sure that you use additional reminder mechanisms, e.g. for your tasks, exercises, medication. For important information about your diagnosis and treatment, please refer to the documents and websites that your doctors and therapists have made available to you.

Important note: As part of the sekTOR-HF innovation fund project, this app supports participating patients in transferring their recorded telemedical data to the eHealth platform. We expressly point out that the use of the app in no way replaces the regular consultation and treatment of a doctor or therapist. Before patients make medical decisions, you should consult with the treating doctor. In the case of new complaints or complications in particular, we recommend that you see your doctor or therapist as soon as possible.
