Stellantis Self-monitoring pro

Description

This application is provided by the Stellantis group to facilitate the administrative procedures related to returning to work sites.

The goal is to protect employees, by asking everyone to carry out self-monitoring. As a reminder, each employee must present a certificate on their honor that this self-monitoring protocol has been carried out and the absence of symptoms over 14 days, in order to be able to access the group's establishments.

The data entered in this application remain entirely the responsibility of the user, and are not transmitted to the Stellantis group or to a third party.
They are stored and accessible only locally on the smartphone.

The medical monitoring screen provided for in the application must only be visually consulted by a doctor or a health professional, by physical presentation of the smartphone and the application by the user. No other person is authorized to view this screen.

The sworn statement screen must be physically presented to the person controlling access to the site.

We recommend that each user secure access to their mobile phone with a password to prevent application data, or any other personal data, from being viewed by other people.

The medical follow-up data is kept for 14 days in the application, then deleted.
Complete erasure of data is possible by uninstalling the application on the phone.

The use of this application is optional, a paper form is also available.
