Getting the healthcare you need should be as convenient as possible. The Memorial Care app offers you just that, thanks to the following easy-to-use features:

Online Appointment Scheduling
Use the Memorial Care app to schedule appointments, both in clinic or through our telehealth option, with our physicians and advanced practice providers, anytime, anywhere.

Urgent Care
You can use the app to locate the nearest Memorial Care clinic offering urgent care services and see how many people are waiting, as well as check in before leaving home using the “On My Way” feature.

If a clinic visit doesn’t fit your schedule, use the app to launch a virtual telehealth appointment. Using this convenient service, you can talk to an experienced Memorial Care provider via secure video link and get a diagnosis and treatment plan without leaving your home.


Patient Portal
Use FollowMyHealth or DMH Chart to access your health records, view test and lab results, request prescription renewals, talk to your doctor and more.

How It Works
The first step is to create a profile within the Memorial Care app. This profile will include your name, birthdate and basic contact info. Your profile also allows you to add dependents and create emergency contacts. Your dependents and emergency contacts can also save their spot in line using your Memorial profile. The app displays all your upcoming and past appointments.
