Description

The Zorgapp ZHN is an app for caregivers in South Holland region with the aim to optimize collaboration by care providers, caregivers and agreements centrally and digitally searchable. In the app you will find locally made transmural working arrangements, the integrated care programs, social security card with the professionals and the opportunity involved in dementia care to register for training. For users with a login code for the 'who's who' have contact fellow caregivers (medical specialists, GPs and other primary care providers, professionals involved in dementia care) accessible.
