Colnec® is an interactive application, available on computer, smartphone or tablet for monitoring the care of people with chronic or complex conditions. Co-built with patients and physicians, this collaborative platform allows the real-time capture of clinical and behavioral data.
The application provides the opportunity for the patient and health professionals to jointly develop a personalized care plan according to the capabilities and preferences of the patient.
On a daily basis, she accompanies the patient in the implementation of a comprehensive and effective treatment:
• Exchange with health professionals between consultations,
• Scheduling appointments and taking medication,
• Hygiene and dietary notifications,
• Reporting of adverse effects,
• Access to reports and medical results,
• Sharing information with health professionals, other patients and relatives (instant messaging, teleconsultation).
