Description
The BARE Toolkit app makes it easy for clinicians to do evidence-based assessment. Combine information to get updated probabilities of diagnoses and treatment targets. The app also benchmarks problem severity and makes it easit to set goals and track daily progress.
The clinician stays responsible for determining any diagnosis and severity of a client based on his/her evaluation. The app makes it simple to gather, score, and interpret the information to guide these decisions.
The app also allows client and clinician to agree upon and set short and long term goals.
Finally, the app has a daily check in for the client to track how they are feeling. The self-reported data is compiled in charts for clinician and client to review.
